📋 Tanggung Jawab Pekerjaan

• Review and interpret medical records to ensure accurate medication orders.
• Collaborate with doctors to optimize medication therapy and minimize adverse drug reactions.
• Administer and monitor intravenous medications, including insulin and chemotherapeutic agents.
• Educate patients and their families about medication use, including proper dosage, side effects, and potential interactions.
• Conduct medication therapy management reviews to ensure patient safety and adherence to treatment plans.
• Stay updated with the latest pharmaceutical research and best practices to provide evidence-based care.
• Participate in quality improvement initiatives to enhance patient outcomes.

🎓 Kualifikasi & Syarat

• Bachelor's degree in Pharmacy.
• Minimum of 5 years of experience as a Clinical Pharmacist in a hospital setting.
• Certification from a reputable pharmaceutical board, such as the Indonesian Pharmaceutical Association (IAP).
• Proficiency in English, both written and spoken.
• Strong communication and interpersonal skills.
• Ability to work in a fast-paced, high-pressure environment.
• Knowledge of electronic health records (EHR) systems, such as Epic or Cerner.
